Make the Connection: Helping Canadians Living with Pulmonary Hypertension Find Support Beyond the Appointment

This November, PHA Canada calls on healthcare professionals to connect patients and caregivers with resources, education, and community support.

Vancouver, BC (November 1, 2026) — A medical appointment is an essential part of pulmonary hypertension care. But when patients return home, questions, emotional challenges, and the demands of daily life continue. This Pulmonary Hypertension Awareness Month, the Pulmonary Hypertension Association of Canada (PHA Canada) is asking healthcare professionals to help bridge that gap.

Under the theme Make the Connection, November’s campaign focuses on raising awareness of pulmonary hypertension (PH) among healthcare professionals and ensuring they know where to find resources and support for their patients.

Living with PH can affect much more than physical health. Patients and caregivers may need help understanding a diagnosis, preparing questions for their care team, navigating everyday challenges, or finding someone who understands their experiences. Knowing where to turn can make a meaningful difference.

PHA Canada offers educational resources, webinars, peer mentoring, support groups, and information for patients and caregivers. These resources complement clinical care, helping people learn about PH and connect with others as they navigate life with the condition.

The campaign encourages physicians, nurses, and other healthcare professionals to explore these resources, share them with patients and caregivers, and make conversations about support a regular part of care. A link shared during an appointment, a resource included in a patient information package, or a recommendation to explore peer support can open the door to further connection.

Healthcare professionals can visit phacanada.ca to learn more and find resources to share with their patients.
